The Surgical Spasticity Program at Riley Children’s Health helps children with movement disorders. These disorders, such as spasticity and dystonia, impact body control and muscle tone. The conditions we treat are often seen in children with cerebral palsy, or who have had a stroke or due to certain metabolic or genetic conditions.
You’ll have a full team of specialists working together to understand your child’s needs. Our team includes experts in the following areas:

What types of movement disorders do we treat in the Surgical Spasticity Program?
Movement disorders in children are common and often result in reduced function and discomfort. They typically stem from brain or spinal injuries, past infections, genetic or metabolic issues, or autoimmune conditions. Symptoms include fine and gross motor delays, limited range of motion in limbs, poor control of body motions, and muscle tightness.
Among the conditions we treat are:
- Spasticity and related complications such as muscle contractures
- Dystonia and dyskinesias
- Mixed movement disorders, commonly seen in conditions like cerebral palsy
- Neuromuscular disorders affecting muscle tone, coordination, and mobility
- Orthopedic complications resulting from neuromotor dysfunction
What to Expect with the Surgical Spasticity Program
When you and your child come to our program, our entire team will meet with you during one visit. That way, you don’t have to go to different locations or make multiple appointments. Instead, at this one appointment, you’ll meet with the whole team, and together with you we’ll come up with the best plan to help your child.

Your child’s care plan may or may not include surgery. Our team may recommend a surgery based on your child’s condition and needs.
Intrathecal Baclofen Therapy (ITB)
Baclofen pumps are devices that make your child’s muscles more relaxed and comfortable. They relieve spasms and muscle cramping. The pump gives medicine directly to the fluid around the spine at scheduled times during the day.
The pump is installed with a short surgery. The pump is placed under the skin in your child’s torso. It’s attached to the spine with a small tube. This lets medicine go straight to where it’s needed.
Deep Brain Stimulation (DBS)
In deep brain stimulation, our neurosurgeons place tiny wires, called electrodes, in specific areas of your child’s brain. These wires are connected to a small device, similar to a pacemaker, which is placed under the skin near the collarbone.
The device sends regular electrical impulses to the target areas of the brain to correct excessive or misguided signals in the brain.
Selective Dorsal Rhizotomy (SDR)
For some patients with cerebral palsy, selective dorsal rhizotomy (SDR) is a surgery that helps reduce spasticity and increases overall functional ability.. The treatment involves cutting (sectioning) problematic nerve roots in the spine to improve muscle symptoms.
Non-selective Dorsal Rhizotomy
Instead of using a baclofen pump, another option for children who need complete care is a surgery called non-selective dorsal rhizotomy. The goal of this surgery is to decrease muscle spasticity that causes pain, makes it hard to clean, or makes sitting in a wheelchair uncomfortable. This is a permanent procedure that helps to decrease the abnormal tone in the legs.
Single-event Multilevel Surgery (SEMLs)
Single-event multilevel surgery (SEMLs) is a type of orthopedic surgery where several muscle and bone problems are fixed in one operation. This is often used for children with cerebral palsy who have trouble walking or moving because of joint and bone issues. By doing everything in one procedure, your child can avoid having multiple surgeries and recovery periods.
